Heart Palpitations: Causes, Red Flags, and How They Are Evaluated

Short answer: Most heart palpitations are harmless extra beats triggered by stimulants, stress, poor sleep, dehydration, or hormonal changes. Seek urgent care if they come with fainting, chest pressure, or severe breathlessness. Diagnosis depends on capturing the rhythm during a symptom — a Holter monitor for daily symptoms, a 7- to 14-day patch monitor for weekly ones.

It arrives without warning. A flutter high in the chest while you are sitting perfectly still. A single heavy thud that feels like the heart skipped a beat and then caught itself. A stretch of racing that begins suddenly, runs for a few minutes, and stops just as abruptly as it started. Palpitations — an awareness of your own heartbeat — are among the most common reasons people come to a cardiology office, and among the most unsettling, because the sensation is impossible to ignore and almost always gone by the time you are sitting in front of a doctor. The point of a proper evaluation is to demonstrate that — and to identify the minority that are not.

What a palpitation actually is

Your heart beats roughly 100,000 times a day and you notice almost none of it. A palpitation is simply a beat, or a run of beats, that breaks through into awareness. People describe the same underlying events in very different ways: fluttering, pounding, flip-flopping, a pause followed by a thump, or a sensation in the throat or neck rather than the chest.

Those descriptions map onto three broad categories:

  • Extra beats. A premature atrial contraction (PAC) or premature ventricular contraction (PVC) fires early, which leaves a compensatory pause before the next beat. That next beat arrives with a fuller ventricle and lands harder — which is why the classic description is a skip followed by a thud. The thud is the beat you feel, not the one you missed.
  • A faster rhythm. Supraventricular tachycardia (SVT) typically switches on and off like a light, often at rates of 150 to 220. Atrial fibrillation feels fast and chaotically irregular, with no discernible pattern at all.
  • A normal heart beating hard. Anxiety, fever, dehydration, anemia, pain, an overactive thyroid, or a stimulant will all raise your heart rate appropriately. The rhythm is normal; the force and speed are what you notice.

The common and usually harmless causes

In a general population, the great majority of palpitations trace back to something ordinary:

  • Stimulants and substances — caffeine, nicotine, alcohol, decongestants containing pseudoephedrine, some asthma inhalers, thyroid replacement that is dosed slightly high, and energy drinks.
  • Stress, anxiety, and panic. This is a genuine physiological cause, not a dismissal. Adrenaline raises heart rate and force, and heightened attention makes normal beats noticeable.
  • Sleep deprivation and dehydration, both of which raise circulating adrenaline.
  • Hormonal shifts — pregnancy, the perimenopausal transition, and the days around a menstrual period.
  • Fever and acute illness.
  • Position. Many people notice extra beats only when lying on the left side at night, because the heart sits closer to the chest wall.

Two common assumptions deserve correction. First, coffee is not the reliable villain it is made out to be. In the CRAVE trial, 100 adults alternated days of unrestricted coffee with days of complete caffeine avoidance while wearing continuous monitors; caffeinated days produced no significant increase in premature atrial contractions. Premature ventricular contractions did increase — roughly 154 per day versus 102 — but chiefly among faster caffeine metabolizers. If cutting caffeine helps you, cut it. If it does not, the evidence does not demand that you give up coffee.

Second, feeling your heartbeat frequently does not mean you have many more extra beats than everyone else. Awareness varies enormously between people. Two patients with identical monitor results can have wildly different symptoms.

The causes that need attention

A smaller share of palpitations reflect an arrhythmia or an underlying heart problem worth finding:

  • Atrial fibrillation. The most common sustained arrhythmia, and the one with the highest stakes, because it raises stroke risk and is frequently treatable. US prevalence is projected to reach about 12.1 million by 2030. It can be entirely asymptomatic, which is precisely why episodes that are felt should be documented.
  • Atrial flutter and SVT. Usually not dangerous, often very treatable — frequently curable with a catheter ablation when episodes are disruptive.
  • A high burden of PVCs. Occasional PVCs are normal. When they make up a large share of total beats, they can gradually weaken the heart muscle. PVC-induced cardiomyopathy is typically considered at a burden of 10 percent or more on ambulatory monitoring, which is a threshold you cannot estimate from symptoms — only from a recording.
  • Structural heart disease. Valve problems, a weakened or thickened heart muscle, or prior heart damage can all present with palpitations.
  • Non-cardiac drivers worth ruling out with simple labs: an overactive thyroid, anemia, and significant electrolyte abnormalities.
  • Untreated sleep apnea, a well-established and modifiable contributor to atrial fibrillation.

Red flags: when palpitations need urgent attention

The sensation itself matters less than the company it keeps. Call 911 if palpitations occur with any of the following:

  • Fainting, or nearly fainting
  • Chest pressure, tightness, or pain
  • Severe or sudden shortness of breath
  • Confusion, one-sided weakness, or slurred speech

Arrange a prompt evaluation — not an emergency visit, but not next year either — if your palpitations are frequent or lasting longer, if they feel fast and irregular, if they begin during exertion rather than at rest, if you already have known heart disease, or if a close relative died suddenly at a young age or has an inherited arrhythmia condition. Palpitations that start with exercise and stop when you rest deserve particular attention, since exertion-triggered arrhythmias behave differently from the ones that show up on the couch.

How palpitations are evaluated

A good workup is mostly about capturing the rhythm while you are feeling the symptom. Everything else supports that goal.

The history. Often the most informative part of the visit. Sudden on-and-off suggests SVT; irregularly irregular suggests atrial fibrillation; an isolated skip-then-thud suggests ectopic beats; a gradual rise and fall over minutes suggests a normal rhythm driven by adrenaline. We also review every medication, supplement, and stimulant, because a surprising number of cases resolve there.

A resting EKG. Quick and worth doing even when you feel fine, because it can reveal a prior heart attack, chamber enlargement, conduction abnormalities, pre-excitation (as in Wolff-Parkinson-White), or a long QT interval.

Blood work. Thyroid function, a complete blood count for anemia, electrolytes, and kidney function.

An ambulatory monitor — matched to how often symptoms occur. This is the step most often gotten wrong. A 24- to 48-hour Holter monitor is the right tool when symptoms happen daily; it is close to useless for symptoms that occur once a week, because the odds of an episode landing inside the recording window are low. For weekly symptoms, a 7- to 14-day adhesive patch monitor is far higher yield. For genuinely infrequent episodes with a serious suspicion behind them, longer-term monitoring is available. Recording duration matters for quantification too: extending from a 24-hour Holter to a 7-day monitor roughly doubles the detection of patients whose PVC burden exceeds 10 percent.

An echocardiogram when the EKG is abnormal, a murmur is present, symptoms are significant, or a high PVC burden is documented. It is a painless ultrasound that shows the valves, the chamber sizes, and how strongly the heart squeezes.

A stress test when palpitations are brought on by exertion, or when coronary disease is a concern alongside them.

What about my smartwatch?

Consumer wearables have genuinely changed this conversation. A single-lead tracing captured on a watch during an episode can be extremely useful, and many patients arrive having already recorded the event that a monitor would have taken weeks to catch. Bring those recordings — export them as PDFs if you can.

Two caveats. Wearable rhythm alerts produce false positives, particularly during motion or poor skin contact, so a watch notification needs medical confirmation before it becomes a diagnosis or a prescription. And a normal watch tracing recorded hours after an episode tells you very little. The recording must overlap with the symptom.

How palpitations are treated

Treatment follows the diagnosis, and for a large share of patients it begins and ends with reassurance plus a few adjustments: reducing stimulants, correcting a thyroid or iron problem, improving sleep, treating sleep apnea, addressing anxiety, and changing an offending medication. Knowing with certainty that the rhythm is benign is, for many people, the treatment itself.

When something more is required, the options are well established. Beta blockers or calcium channel blockers reduce symptomatic extra beats and control rate. Atrial fibrillation is managed with rate or rhythm control alongside a formal stroke-risk assessment and anticoagulation when indicated. SVT and high-burden PVCs that resist medication can often be cured with catheter ablation. And when palpitations arise from an underlying issue — a valve problem, coronary artery disease, or a weakened heart muscle — treating that condition is what resolves the symptom.

Are heart palpitations dangerous?

Most are not. The majority of palpitations come from extra beats, stimulants, stress, dehydration, poor sleep, or hormonal changes, and they occur in structurally normal hearts. Palpitations are more concerning when they come with fainting, chest pressure, or severe shortness of breath — which warrant calling 911 — or when they are frequent, feel fast and irregular, start during exertion, or occur in someone with known heart disease or a family history of sudden death at a young age.

What is the difference between a skipped beat and atrial fibrillation?

A skipped beat is usually a premature contraction: the heart fires early, pauses, and the following beat lands harder, producing the classic skip-then-thud sensation. It is brief and isolated. Atrial fibrillation feels different — fast, chaotic, and irregular with no pattern, often lasting minutes to hours, and sometimes accompanied by breathlessness or fatigue. Only an EKG or a heart monitor recorded during the episode can tell them apart reliably.

Does caffeine cause heart palpitations?

Less than most people assume. In the CRAVE trial, participants alternated days of unrestricted coffee with days of complete caffeine avoidance while wearing continuous monitors, and caffeinated days produced no significant increase in premature atrial contractions. Premature ventricular contractions did rise modestly, mainly in faster caffeine metabolizers.
