Few numbers on a lab report cause as much confusion as cholesterol — and few medications are argued about as much as statins. Patients arrive at our office having read that cholesterol is the whole story, or that it is meaningless; that statins are essential, or that they are handed out too freely. The truth is more useful than either extreme, and it changed meaningfully in 2026, when a new national guideline replaced the one that had guided cholesterol care since 2018. At Northwest Cardiovascular Clinic, we walk patients from Tomball, Cypress, Spring, and across northwest Houston through their numbers every day. Here is what they actually mean, who benefits from a statin, and what to do about side effects.
What is on a cholesterol panel — and what each number means
Cholesterol itself is not a villain. Your body needs it to build cell membranes, hormones, and vitamin D. The problem is transport. Cholesterol travels through the blood inside particles, and some of those particles slip into artery walls and start plaque. A standard lipid panel reports four numbers:
- LDL cholesterol — the cholesterol carried by low-density lipoproteins. These are the particles that deposit into artery walls, which is why LDL is the primary treatment target and the number most worth lowering.
- HDL cholesterol — carried by high-density lipoproteins, which help move cholesterol back out. Higher tends to be better, but raising HDL with medication has never been shown to prevent heart attacks, so it is best read as a marker rather than a target.
- Triglycerides — a different kind of blood fat, strongly influenced by alcohol, refined carbohydrates, excess weight, and untreated diabetes. Very high levels also risk inflaming the pancreas.
- Total cholesterol — the least useful number of the four, because it lumps the helpful and the harmful together.
A fifth number, non-HDL cholesterol, is simply total cholesterol minus HDL. It captures every artery-clogging particle in one figure, and it does not require fasting — which is why it has become a formal treatment target alongside LDL.
Two newer numbers worth asking about
ApoB counts the actual number of atherogenic particles rather than the cholesterol packed inside them. Two people can share an identical LDL while one carries far more particles, and the one with more particles carries more risk. ApoB is most informative when triglycerides are above 200 mg/dL, when you have diabetes, or when your LDL already looks good but your risk still feels high.
Lipoprotein(a), written Lp(a), is inherited, largely unaffected by diet or exercise, and stays roughly stable for life — so a single test answers the question permanently. Current guidance recommends measuring it at least once in every adult’s lifetime. A level at or above 125 nmol/L (about 50 mg/dL) is associated with roughly 1.4 times the cardiovascular risk, and 250 nmol/L or higher with about double. An elevated Lp(a) does not have a dedicated treatment yet, but it changes the plan: it argues for lowering LDL harder, controlling blood pressure tightly, and testing close relatives.
What the 2026 guideline changed
In March 2026, the American College of Cardiology and American Heart Association, with eleven partner organizations, released a new dyslipidemia guideline that retired the 2018 cholesterol guideline. Three changes matter most to patients:
Numeric goals are back. The 2018 guideline emphasized percentage reduction; the 2026 version restores explicit LDL and non-HDL targets. For people who already have cardiovascular disease and are at very high risk — which describes most people who have had a heart attack, stent, bypass, or stroke — the goal is an LDL under 55 mg/dL and a non-HDL under 85 mg/dL. Those with established disease but not at very high risk have a goal of at least under 70 mg/dL.
Risk is calculated differently. The older Pooled Cohort Equations have been replaced by the AHA’s PREVENT equations for adults aged 30 to 79. PREVENT estimates both 10-year and 30-year risk, incorporates kidney function and metabolic health, and removed race as a predictor. For someone in their forties, a 30-year number is often the more honest picture.
Treatment starts earlier. The guideline explicitly favors reducing lifetime exposure to these particles rather than waiting for risk to accumulate. It advises considering medication in young adults with an LDL of 160 mg/dL or higher, or with a strong family history of early heart disease.
Who actually needs a statin
Statins are not for everyone, and the decision is rarely automatic. Broadly, treatment is recommended or considered in these situations:
- You already have cardiovascular disease — a prior heart attack, stent, bypass, stroke, coronary artery disease, or peripheral artery disease. Here the benefit is largest and least debatable.
- Your LDL is 190 mg/dL or higher. This often signals familial hypercholesterolemia, an inherited condition affecting roughly 1 in 250 people. High-intensity statin therapy is recommended regardless of any risk score, and first-degree relatives should be screened.
- You are 40 to 75 with diabetes, chronic kidney disease stage 3 or 4, or HIV — LDL-lowering therapy is recommended regardless of your starting LDL.
- Your calculated risk warrants it. With a 10-year PREVENT risk of 5 percent to under 10 percent, therapy should be considered after a real conversation; at 3 to under 5 percent, it can be considered. Below that, lifestyle usually leads.
When the decision genuinely sits on the fence, a coronary artery calcium score often resolves it. It is a quick, non-contrast CT that measures calcified plaque directly. A score of zero may justify waiting and rechecking; a high score usually settles the question. We describe how this fits into a broader workup in our guide to preventive heart screening.
How statins work, and how well
Statins block an enzyme the liver uses to make cholesterol. The liver responds by pulling more LDL out of the bloodstream, which is where most of the benefit comes from. They also appear to stabilize existing plaque, making it less likely to rupture and cause a heart attack.
The magnitude is well established: across large randomized trials, every 39 mg/dL (1 mmol/L) drop in LDL corresponds to roughly a 20 to 25 percent lower annual risk of major cardiovascular events, and the benefit accumulates the longer treatment continues. High-intensity therapy — atorvastatin 40 to 80 mg or rosuvastatin 20 to 40 mg — typically lowers LDL by 50 percent or more. Moderate-intensity doses lower it by about 30 to 49 percent.
What matters is that absolute benefit depends on your starting risk. A 25 percent reduction is substantial for someone who has already had a heart attack and modest for someone whose risk was low to begin with — which is exactly why the risk calculation comes before the prescription.
Statin side effects: what the evidence actually shows
This is where most of the anxiety lives, and it deserves a straight answer.
Muscle aches. Muscle symptoms are the most common reason people stop statins, and they are real experiences — but they are frequently not caused by the drug. The SAMSON trial gave 60 patients who had previously quit statins alternating months of atorvastatin, identical placebo, and no tablet at all. Symptom scores on placebo were nearly identical to those on the statin, and both were higher than on no tablet. About 90 percent of the symptom burden was attributable to the act of taking a pill rather than to the statin itself. Notably, half of the participants successfully restarted treatment afterward. Serious muscle injury (rhabdomyolysis) does occur, but it is rare — on the order of one to three cases per 100,000 patient-years.
Blood sugar. Statins modestly increase the chance of being diagnosed with type 2 diabetes, largely in people already close to that threshold. In pooled trial data this amounts to roughly one additional diagnosis per 250 people treated over about four years — substantially outweighed by the cardiovascular events prevented in anyone with meaningful risk.
Liver. Clinically important liver injury is rare. Mild enzyme elevations occur and usually resolve. Routine repeat liver testing is no longer required for most patients.
Memory. Despite persistent claims online, controlled trials have not demonstrated that statins cause cognitive decline.
If you feel unwell on a statin, the answer is a conversation, not silent discontinuation. Rechecking on a lower dose, switching agents, trying alternate-day dosing, or a deliberate rechallenge resolves the great majority of cases.
When a statin is not enough — or not tolerated
Several non-statin options now exist and are supported by outcome data. Ezetimibe is an inexpensive daily tablet that blocks cholesterol absorption in the gut. Bempedoic acid works upstream of the statin target and is often better tolerated by people with muscle symptoms. PCSK9 inhibitors are injections given every two to four weeks that can lower LDL dramatically, and inclisiran achieves similar lowering with a twice-yearly injection after the loading doses. These are typically added for people with established disease or very high LDL who are not reaching goal.
What still matters besides medication
No prescription replaces the basics, and several of them move LDL and triglycerides measurably: a dietary pattern built on vegetables, fruit, whole grains, legumes, nuts, fish, and olive oil; soluble fiber from oats, beans, and psyllium; replacing butter and processed meat with unsaturated fats; about 150 minutes a week of moderate activity; stopping nicotine in every form; treating sleep apnea; and modest weight loss when it applies. Triglycerides in particular respond quickly to cutting alcohol and refined carbohydrates.
When to seek care
High cholesterol causes no symptoms — that is precisely the problem. But do not wait on symptoms that suggest the arteries are already involved: chest pressure or tightness with exertion, new breathlessness, cramping calf pain when you walk, or unexplained leg swelling all deserve evaluation. And call 911 for chest pressure lasting more than a few minutes, sudden severe shortness of breath, fainting, or one-sided weakness or slurred speech.

Whether lifestyle alone is enough depends on how far your numbers are from goal and how high your overall risk is — which is a decision to make with your physician rather than alone.
